- A valid concession card holder like the Health Care Card, Pensioner Concession Card, or Low Income Health Care Card.

- Inform our dentist in St Clair that you have been referred from a public dental clinic and require treatment under OHFFSS.
- Bring proof of identity, eligibility, and the Oral Health Treatment Authority Voucher to your dentist appointment.
- On your part, you will need to certify the satisfactory completion of your treatment on the issued voucher once your dental work from our dentist is completed.
- Any additional treatments outside of the voucher’s limitations will need to be discussed with one of our dentists at Your Healthy Smile dental clinic.
